The enumeration date is reported by NPPES. A recent enumeration can support a “newly enumerated” signal but does not prove a practice opened.
First observed is ProviderFront’s own longitudinal milestone. It is not the same as the official enumeration date.
Taxonomy codes categorize reported provider type and specialization. Multiple codes may be current, and they do not establish credentials or quality.
An NPI is a ten-digit identifier issued through NPPES. Issuance does not validate licensure, credentialing, quality, network participation, or patient availability.
NPPES is the National Plan and Provider Enumeration System. ProviderFront uses official downloadable files for longitudinal ingestion and the NPPES API for targeted freshness checks.
This is an inference label, not confirmation. ProviderFront shows the factors and avoids unsupported precision.
Suppression can apply to a provider record or a sensitive location. Active suppression survives later source imports until reviewed.
